The Resettlement Health Program (RHP) Nurse will provide clinical support to providers during the initial health assessment and follow-up visits for newly arrived Humanitarian Immigrants, in accordance with the requirements set forth by the Maryland Department of Health (MDH) Office of Immigrant Health. The RHP Nurse is a Registered Nurse responsible for coordinating the clinical activities of the Refugee Health Program, including but not limited to overseeing the delivery of clinical care during Refugee Health Assessment (RHA), pre-vite planning, mental health screenings, as well as implementing care plans and conducting health education for Humanitarian immigrants. The RHP Nurse will serve as the lead for the Refugee TB program at BMS and will report to the Director of International Services. Within the Patient-Centered Medical Home (PCMH) model, the RHP Nurse will work with the RHP Manager to ensure that all core activities outlined in the agreement with MDH are performed and will follow MDH and the relevant Centers for Disease Control (CDC) guidelines.

Essential Functions:
- Assess care management patients initially, periodically, and at critical junctures for comprehension of their clinical condition, understanding of important steps in the care plan, confidence in self-management, motivation, and ability to persevere in the face of emotional and other obstacles.
- Collects health status data relevant to the patient’s present problem in a continuous, systematic, and complete manner. Completes physical assessment and correlates with a patient's health history to prepare the patient for examination or treatment as appropriate.
- Assures the provision of teaching/ learning opportunities for the patient. Conducts patient/ family/ significant other educational assessment and develops a teaching plan, including educational material design.
- Develops an interactive care plan with the patient and the care team.
- Conducts panel reviews using registries and data dashboards to coordinate and plan care for patients with chronic illnesses.
Requirements
- 2+ years of experience as an RN in a family practice or community health setting
Required: Graduate of an accredited school of professional nursing by date of hire.
Preferred: BSN
- Registered Nurse in the State of Maryland (or compact state)
- Unexpired CPR certification from the American Heart Association or American Red Cross
- Spanish speakers preferred
