In this role, you will take the lead in assessing and managing wounds, supporting staff education, and ensuring best practices are followed. Key Responsibilities:

  • Conduct thorough assessments of all wounds, pressure injuries, ulcers, surgical sites, and other skin conditions.

  • Develop and implement individualized care and treatment plans based on clinical findings and physician orders.

  • Perform wound cleansing, dressing changes, and advanced wound therapies in accordance with established protocols.

  • Monitor wounds for signs of infection, deterioration, or complications and report changes promptly to the appropriate provider.

  • Accurately document wound assessments, treatment interventions, and progress notes in compliance with facility policies and regulatory requirements.

  • Educate residents, families, and staff on wound prevention, skin care techniques, and infection control measures.

  • Collaborate closely with physicians, nurse leaders, and other members of the care team to ensure continuity and consistency of treatment.

  • Participate in quality assurance and performance improvement initiatives related to wound prevention and management.

  • Maintain up-to-date knowledge of best practices, products, and procedures in wound care.

  • Promote a positive, compassionate, and professional atmosphere that reflects the values and mission of Collierville Nursing & Rehabilitation. 


LPN/RN Wound Nurse Licenses/Certification:

  • Current, valid LPN or RN license in the state.

  • Wound Care Certification.

  • Prior wound care experience in a long-term care or skilled nursing setting is required.
