Quality Intellectual Disability Professional (QIDP)
Location: New Hope Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/ID)
Position Type: Full-Time
Are you passionate about making a meaningful difference in the lives of individuals with intellectual and developmental disabilities? Join our dedicated leadership team at New Hope's ICF/ID as a Quality Intellectual Disability Professional (QIDP).
As a QIDP, you will play a vital role in coordinating and overseeing individualized services and supports for approximately 18 adults. You will be responsible for developing and managing comprehensive care plans, advocating on behalf of the individuals you serve, and acting as a liaison between the facility, families, and external funding agencies.
Key Responsibilities:
Coordinate and monitor person-centered service plans for assigned individuals
Advocate for the rights, needs, and preferences of each person served
Collaborate with interdisciplinary teams, families, and funding sources
Ensure compliance with all regulatory and organizational standards
Maintain accurate documentation and progress reports
Requirements
Qualifications:
Bachelor’s degree in a human services field or licensure as a Registered Nurse (RN)
Minimum of 1 year of experience working with individuals with intellectual or developmental disabilities
Strong organizational, communication, and interpersonal skills
Self-motivated and capable of managing multiple priorities
