  • M - Thurs,- 8am - 4pm & Fridays 8am - 3:30pm (No weekends)

Job Description: 

  • Plans and coordinates a schedule of recreational and leisure time activities including a monthly food order and activity calendar with a minimum of four activities per program day.
  • Carries out planned recreation programs.
  • Monitors and maintains supplies necessary for program implementation within the day program recreation budget.
  • Assesses recreational and leisure time needs of each participant by fifth program day and not to exceed 30 days, develops measurable and individualized goals, implements meaningful activities and completes a written evaluation of outcomes for each participant every six months.
  • Functions as part of an interdisciplinary team including attendance at or providing documentation for care planning conferences.
  • Keeps attendance records for activities and end of month stats and document encounters / activities in EHR
  • Plans and organizes community field trips/ community integration events.
  • Trains and supervises program assistants, volunteers and students in the implementation of activities.
  • Performs other tasks necessary for the effective operation of recreational programs as assigned by the director including but not limited to budgets, community connection, newsletter.  
  • Durand LEA will support the social day program with admission and routine assessments, quarterly progress notes and NHTD daily notes.
  • Keep work area clean, clear and safe. 

Candidate Requirements:

  • Bachelor's degree
  • 1 - 2 years active participation in the delivery of organized recreational activities with geriatric experience preferred; or equivalent combination of education and experience.
  • Therapeutic Recreation Specialist (CTRS) preferred.
