Job Description
This temporary role is expected to continue through August 31, 2026, at which time the assignment will end. The Intake Case Manager is a critical first point of contact for individuals and families entering Ascencia’s Access Center. This position is responsible for conducting initial screenings, assessing client needs, coordinating with case managers, and supporting Access Center staff in delivering high-quality, client-centered services. The Intake Case Manager ensures that clients receive accurate information, referrals, and timely access to programs in alignment with Ascencia’s service standards.

Salary: $24.00 per hour
Duties and Responsibilities
Access Center / Intake
Serve as the primary “first contact” for clients entering the Access Center.
Greet clients and visitors, providing program information and resource referrals.
Answer phones and direct calls appropriately.
Maintain a clean and welcoming waiting area.
Conduct initial intake and needs assessment interviews.
Collect, verify, and update client information in case files and HMIS.
Monitor and ensure availability of case management forms and office supplies.
Evaluate client histories, coordinate consultations, and recommend client eligibility to case managers.
Facilitate coordination of case management objectives in partnership with primary case managers.
Maintain accurate documentation of services, outcomes, and related activities in compliance with federal, state, county, and agency guidelines.
Oversee client mail distribution, ensuring adherence to Mail Distribution policies.
Client Support & Engagement
Assist clients in accessing shelter, housing, and supportive resources.
Conduct crisis intervention and provide guidance in urgent situations.
Ensure that case management practices are client-centered and aligned with Ascencia’s standards.
Administrative
Actively participate in staff meetings, trainings, and CES initiatives.
Assist case managers in maintaining administrative records and reporting systems.
Prepare reports on case management activities, outcomes, and program successes.
Maintain professional boundaries and adhere to ethical standards in client interactions.
Qualifications and Requirements
Associate degree in human services or comparable combination of education and work experience preferred.
Experience in a social service setting with knowledge of case management systems and planning techniques.
Knowledge of intake procedures, CES, and the Continuum of Care for homeless persons.
Experience working with homeless individuals and families preferred.
Knowledge of HMIS strongly preferred.
Bilingual (Spanish/English) highly desirable.
Must meet insurance requirements to drive agency vehicles.
Must possess a valid driver’s license and maintain active auto insurance.
Reliable transportation for job-related travel required.
Physical Requirements
Ability to travel for job-related duties, including client meetings and community outreach.
Use of computer and phone for documentation and communication.
