Job Title: Home Health Aide (HHA) - Escort
Facility: Briarcliff Manor Center for Rehabilitation and Nursing Care
Location: Briarcliff Manor, NY
Shift: Full-Time, Morning Shift (7:00 AM – 3:00 PM)
Salary: $20 per hour

About the Role

Join our compassionate team at Briarcliff Manor Center as a Home Health Aide Escort, where you will play a vital role in supporting our residents by escorting them safely to and from clinic and dialysis appointments. This position offers meaningful work with a structured morning schedule and the opportunity to make a positive impact every day.

Responsibilities:

  • Escort residents to and from clinic appointments and dialysis.

  • Arrive at work one hour prior to scheduled clinic appointment.

  • Maintain full nursing uniform (as designated) and wear ID badge both at the facility and in the clinic.

  • Assist residents in preparing for their appointments.

  • Ensure residents receive out-on-pass snacks.

  • Monitor residents to prevent falls and altercations, intervening immediately to prevent incidents.

  • Report all incidents and accidents to the Clinical Coordinator or ADNS immediately.

  • At the clinic, obtain appropriate reports regarding the resident’s visit, ensure they have the necessary paperwork and supplies.

  • Be knowledgeable of residents' DNR/DNI status, ensuring all DNR’s have a non-hospital DNR form with them.

  • Proactively advocate for residents at the clinic to ensure all needs are met and communicate challenges to the facility as needed.

  • Represent Briarcliff Manor Center at all times, adhering to facility policies and procedures, as well as abuse laws, both on and off facility property.

  • Additional clinic visits may be required depending on facility needs.

  • Advise the Clinic Coordinator between appointments if assigned to unit staff duty.

Requirements:

  • Current NYS Home Health Aide (HHA) certification required.

  • CNA certification preferred.

  • Ability to perform all tasks and duties of a Home Health Aide.

  • Strong communication skills, including the ability to read, write, speak, and understand English.

  • Knowledge and skills to provide care appropriate to the age-related needs of residents.
