What You Will Do

  • Provide direct resident care services while following individualized plans of care
  • Administer medications in accordance with all standards of practice, state regulations, and community policies, including proper documentation and follow-up
  • Perform resident assessments as required by state regulations and community policies under the direction of the Director of Assisted Living
  • Observe residents for changes in condition and report concerns promptly to leadership
  • Document all relevant observations, communications, clinical procedures, and daily care by the end of each shift
  • Maintain and update individualized care plans for each resident
  • Support a positive, team-focused environment and assist with additional duties as assigned

Supervisory Responsibilities

This position reports to the Director of Assisted Living and/or Director of Memory Care. While working independently, the LPN is expected to exercise sound judgment and may assist with supervising, directing, and reviewing the work of other associates. Participation in performance evaluations and disciplinary processes may also be required.

Why You Are Qualified

  • High school diploma or equivalent (GED) required
  • Active Licensed Practical Nurse (LPN) license in the state of Tennessee required
  • Current CPR certification required
  • Minimum of 1 year of related experience preferred
  • Ability to perform physical tasks throughout the day, including standing, walking, bending, and assisting residents
  • Must be able to lift up to 25 pounds unassisted and assist with resident transfers using proper equipment
