Job Description

We are currently looking for a Full Time Community Health Representative to join our Ada Area Office Section team located at the Youth & Family Building working .

Summary:
Supports individuals and groups by educating and providing resources that promote a healthy lifestyle.

Responsibilities:
* Provides first class customer service - courteous and helpful to all internal and external customer types
- Answers questions in a courteous and professional manner
- Greets and engages with a smile
- Determines needs, seeks positive experience and outcomes
* Completes all training and development requirements and participates in continuing education opportunities
* Works in a collaborative and supportive manner to support the productivity and safety of the work area, team members and customers of all types
- Maintains general upkeep of work area and maintains a safe environment
- Maintains and promotes professionalism
- Punctual and consistent in meeting start time, work schedule, attendance and other assigned work time metrics
- Adheres to Chickasaw Nation dress code standards
* Maintains knowledge of and follows all laws, rules, regulations, policies and SOPs for assigned area, e.g. MICS, TICS, hospital standards, health codes, food sanitation, accounting principles, etc.
* Maintains confidentiality, security, and integrity of data and information in accordance with HIPAA regulations
* Identifies, coordinates, and connects individuals to available resources
* Assists in scheduling appointments and travel arrangements for patient if necessary
* Performs home visits to complete needs assessment for client’s well-being
* Be available to assist with disaster relief
* Provides an annual rabies clinic
* Advocates and participates in patient and patient family education i.e., diagnosis, medications and discharge instructions
* Records care given and reports patient care observations
* Maintains and documents intake and output
* Performs basic home healthcare services such as; blood pressure, temperature, pulse rate, respiration, blood glucose levels and fall risks
* Provides recommendations/referrals according to clients need assessment for internal and external services
* Transports Chickasaw citizens and other First Americans to and from appointments, meetings, and activities as needed
* Performs all other duties as assigned

* Able to read, understand, apply, and retain knowledge of Chickasaw Nation, departmental, divisional and all other rules, regulations, policies, procedures and guidelines
* Able to verbally communicate in a professional and positive manner with other team members, departments, guests and management


Skills:
Essential:
* Must be familiar with the Microsoft Office Suite of products

Experience:


Nonessential:
* 3 years directly related experience

Education:
Essential:
* High School, GED or Equivalent


Credentials:
Essential:
* Pass general background check
* Cardiopulmonary Resuscitation Certification (CPR)
* Standard Driver's License
* First Aid Certification
* Pass medical administration test
