Description

Function as a Psychiatric Mental Health Nurse Practitioner (PMHNP) in a team and mission-based setting to provide direct psycho-diagnostic, psycho-therapeutic treatments, psycho-pharmacological evaluation management, and therapy to primary care and psychiatric patients in the health center.  PMHNP will diagnose, conduct therapy (when appropriate), and prescribe medications to patients who have psychiatric disorders, medical organic brain disorders, or substance abuse problems. PMHNP will use medical judgment as required to determine the appropriate procedure within the scope of practice.  


Duties/Responsibilities: 

  • Adheres to the spirit of Waterfall Community Health Center to remove barriers to care and our mission statement while performing assigned duties. 
  • Demonstrate the core values of Integrity, Respect, Accountability, Compassion, Partnership, and Collaborative Communication. 
  • Provide psychiatric evaluations and medication management to patients and make timely notations in patient electronic medical records. 
  • Provide ongoing risk assessment to ensure patient health and safety. 
  • Diagnose, conduct therapy, and prescribe medications for patients as appropriate.   
  • Provides case management services for identified patients. Gives primary care providers timely feedback about the patient’s care, treatment recommendations, and progress via documentation in the client’s electronic health record and verbal feedback. 
  • Maintain professional affiliations and enhance professional development to remain abreast of current healthcare management trends and FQCH regulations. 
  • Participate in Clinical Quality Improvement and Quality Management meetings and other meetings as requested by the supervisor and/or Senior Leadership Team. 
  • Adapts therapeutic strategies to individual characteristics of the patient, including but not limited to, disability, gender, sexual orientation, developmental level, culture, ethnicity, age, and health status as measured by return visits and client satisfaction.  
  • Performs any other duties needed to help drive our Vision, fulfill our Mission, and abide by our Organization’s Values.
  • This position may have additional opportunities to perform program management or clinical supervision duties.
Requirements
  • Bachelor's degree from an accredited school of nursing: Master's degree in nursing; completion of a state-acceptable Nurse Practitioner's program; a minimum of one year’s experience is desirable. National certification in specialty is preferred. 
  • Willingness to work a flexible schedule when circumstances are necessary. 
  • Ability to communicate effectively in English, both verbally and in writing. 
  • Maintain confidentiality in accordance with WCHC policies and HIPAA regulations.
  • Program management and/or clinical supervision experience strongly preferred.

LICENSURES/CERTIFICATIONS REQUIREMENT: 

  • Licensed by the State of Oregon as a Registered Nurse, certified by the State of Oregon as a Psychiatric-Mental Health Nurse Practitioner-Board Certified (PMHNP-BC) and Prescriptive privileges. 
  • Ability to be credentialed with Medicaid, Medicare, private and other insurance. 
  • Current American Heart Association (AHA) Provider Basic Life Support Card (BLS) within 2 weeks of start date. 
  • Non-violent Crisis Intervention Comprehensive training class completed within 6 months of hire and recertify prior to the expiration date.  

Immunizations Required: 

TB (test upon hire)  


Physical Requirements: 

  • Prolonged periods sitting, bending and/or leaning over patients, or standing at a workstation using a computer.  Repetitive use of hands and handheld tools of the trade. 
  • Able to lift up to 20 pounds at times. 
  • Able to travel as needed.
