Job Description

Accurate Home Care, LLC is growing in the area of Home Health. We are seeking a Physical Therapist to support patient centered care.  This position acts as our patients’ partner and advocate on their road to recovery, from diagnosis to the completion of rehabilitation. The Physical Therapist will work with Accurate Home Care who maintains multiple assisted living partnerships and provides patient care in collaboration with in house MD services.

Responsibilities:





  • Perform initial and ongoing physical therapy evaluations according to company policy and procedures.




  • Assess, develop and implement a plan of care to promote optimum function and safety.




  • Develop a treatment plan for each patient based on initial and ongoing evaluations including short and long-term goals, collaboration demands of other staff members which will require patient and caregiver involvement.




  • Document patient progress according to approved medical documentation and facility documentation procedures.





Facilitate communication and serves as a liaison between and among nursing and direct staff, family and/or guardians, and community medical providers and professional consultants in cooperation with operations management.
