Job Title: MDS Coordinator (RN/LPN) – Skilled Nursing / Long-Term Care

Now Hiring: MDS Coordinator (RN or LPN) – Skilled Nursing Facility

Haven on the River is actively hiring an experienced MDS Coordinator (RN/LPN) with a strong background in skilled nursing, long-term care, and Medicare/Medicaid reimbursement. This role is essential to ensuring accurate MDS 3.0 assessments, PDPM optimization, and regulatory compliance.

If you are a Registered Nurse (RN) or Licensed Practical Nurse (LPN) with MDS experience in a nursing home or rehab setting, we encourage you to apply.

Key Responsibilities – MDS Coordinator (RN/LPN):

  • Complete and oversee MDS 3.0 assessments (OBRA, PPS, PDPM) in a skilled nursing facility (SNF)

  • Ensure compliance with CMS, Medicare, Medicaid, and state regulations

  • Coordinate with the interdisciplinary team (IDT) for accurate care planning

  • Manage MDS scheduling, ARDs, and timely submissions

  • Monitor case mix index (CMI) and support reimbursement accuracy

  • Maintain audit-ready documentation for survey readiness and compliance

  • Participate in QAPI (Quality Assurance & Performance Improvement) initiatives

  • Assist with Medicare and Medicaid documentation reviews

  • Perform additional administrative duties as assigned by the Director of Nursing (DON)

Qualifications:

  • Active RN or LPN license (required)

  • MDS Coordinator experience in skilled nursing or long-term care (preferred)

  • Strong knowledge of MDS 3.0, PDPM, Medicare, and Medicaid reimbursement

  • Experience with electronic medical records (EMR/EHR) systems

  • Strong organizational, analytical, and time management skills

  • Ability to work independently and collaboratively in a fast-paced healthcare environment.
