Haven on the River is actively hiring an experienced MDS Coordinator (RN/LPN) with a strong background in skilled nursing, long-term care, and Medicare/Medicaid reimbursement. This role is essential to ensuring accurate MDS 3.0 assessments, PDPM optimization, and regulatory compliance.

Key Responsibilities – MDS Coordinator (RN/LPN):
Complete and oversee MDS 3.0 assessments (OBRA, PPS, PDPM) in a skilled nursing facility (SNF)
Ensure compliance with CMS, Medicare, Medicaid, and state regulations
Coordinate with the interdisciplinary team (IDT) for accurate care planning
Manage MDS scheduling, ARDs, and timely submissions
Monitor case mix index (CMI) and support reimbursement accuracy
Maintain audit-ready documentation for survey readiness and compliance
Participate in QAPI (Quality Assurance & Performance Improvement) initiatives
Assist with Medicare and Medicaid documentation reviews
Perform additional administrative duties as assigned by the Director of Nursing (DON)
Qualifications:
Active RN or LPN license (required)
MDS Coordinator experience in skilled nursing or long-term care (preferred)
Strong knowledge of MDS 3.0, PDPM, Medicare, and Medicaid reimbursement
Experience with electronic medical records (EMR/EHR) systems
Strong organizational, analytical, and time management skills
Ability to work independently and collaboratively in a fast-paced healthcare environment.
