Key Responsibilities:

  1. Assist in coordinating and implementing social services programs for residents
  2. Support residents and families in adjusting to facility life and addressing concerns
  3. Help develop and maintain care plans that align with resident needs and preferences
  4. Facilitate resident and family meetings to provide guidance and assistance
  5. Provide resources for financial, emotional, and legal support as needed
  6. Assist with discharge planning and ensure a smooth transition for residents returning home or to another care setting
  7. Maintain accurate documentation and reports in compliance with facility policies and regulations


Qualifications:

  1. Experience in social services, preferably in a healthcare or long-term care setting
  2. Strong understanding of resident rights and care planning processes
  3. Excellent communication and interpersonal skills
  4. Ability to work compassionately with residents, families, and interdisciplinary teams
  5. Associate's or Bachelor's degree in Social Work or a related field is preferred
