We are seeking a dedicated, solutions-focused Director of Social Services to lead and oversee all aspects of resident support, advocacy, and care coordination. This role is about more than meeting regulatory standards—it’s about driving positive outcomes, fostering meaningful connections, and ensuring every resident and family receives exceptional support throughout their care journey. 

Lead and manage the Social Services department, ensuring compliance with all state, federal, and facility regulations

Develop, implement, and monitor individualized care plans in collaboration with the interdisciplinary team

Serve as an advocate for residents’ rights, needs, and preferences

Provide counseling and guidance to residents and families during admissions, care planning, transitions, and discharge processes

Coordinate community resources, referrals, and support services to meet individual needs

Train, mentor, and supervise Social Services staff, promoting professional development and high performance

Maintain accurate documentation and records in accordance with regulatory and organizational standards
