Performs moderately complex social services work. Work involves interviewing clients and others to obtain
information to determine client eligibility. Serves as the point of contact for individuals (or LAR) seeking
mental retardation services. Functions as a liaison between clients (or LAR) and internal and external
providers. Works under moderate supervision with latitude for use of initiative and independent judgment.
Example of Work Performed
· Provides information regarding center and other community services, refers to community resources,
and when appropriate facilitates the admission process.
· Gathers historical, social, functional, psychiatric, developmental or any other information from the
individual (or LAR) seeking MR services in order to assist Psychologist in determining eligibility.
· Interviews client (or LAR) to gather information used in determining client eligibility for financial,
medical, and/or social services.
· Contacts sources to obtain or verify case data used in determining service eligibility.
· Explains program benefits and requirements to client (or LAR).
· Describes other community services available to the client and makes referrals as needed.
· Coordinates DMR/Endorsement process w/ client/LAR and Psychologist.
