Job Description

The Faith and Community Health Navigator works to improve outcomes by referring targeted individuals to appropriate community-based health care, mental health and substance abuse services and serves as a liaison to coordinate and leverage existing community-based resources to improve quality of care and patient engagement and decrease costs for high-risk populations. The position has direct client responsibilities and travels outside of the hospital setting to follow up on patient status. Represents BHCs mission of helping people throughout life's journey by helping provide access and excellent holistic care to all persons. Models appropriate behavior as exemplified in BHC Mission, Vision and Values.

Responsibilities

  • Develops a network of congregations that are committed to attending to health issues.
  • Works cooperatively with faith community representatives to build well-integrated health ministries.
  • Collaborate with patients to develop and implement a plan to access preventive and follow-up healthcare.
  • High risk patient care management.
  • Improves access to care and care received within and for the community.
  • Maintain and document accurate data related to the program.
  • Coordinates community health programs, activities, and events for targeted population.

Qualifications

Minimum Education

  • High School Diploma or Equivalent Required
  • Technical Diploma/Certificate Medical Assisting Required


Minimum Work Experience

  • 1-3 years clinical experience. Required
  • 1-3 years prior experience working in community outreach programs. Preferred


Licenses and Certifications

  • Certified Medical Assistant (CMA_AAMA) Upon Hire Required or
  • Medical Technologist (MT_AMT) Upon Hire Required or
  • Medical Assistant (NCMA_NCCT) Upon Hire Required or
  • BLS for Healthcare Providers (BLS) American Heart Association Upon Hire Required
