Job Description

Job Summary
We are seeking a compassionate and dedicated Hospice Certified Nursing Assistant (CNA) to join our team. In this role, you will provide essential care and support to patients in their homes or hospice facilities, ensuring comfort and dignity during their end-of-life journey. You will work closely with nursing staff and family members to deliver high-quality care tailored to each patient’s needs.

Current opening:

56 hours biweekly, day shift. Occasional weekend and holiday hour availability required, depending on client needs.

Responsibilities
- Assist patients with daily living activities, including bathing, dressing, grooming, and feeding.
- Monitor and record vital signs such as temperature, pulse, and respiration.
- Provide emotional support and companionship to patients and their families.
- Assist with medical documentation as required by nursing staff.
- Help maintain a clean and safe environment for patients.
- Collaborate with healthcare professionals to develop individualized care plans.
- Provide care for patients with specific needs such as dementia or critical care requirements.
- Ensure adherence to safety protocols and infection control measures.

Requirements

Current CNA certification with experience in hospice, palliative, or aging service preferred and committed to embracing St. Paul Elder Services’ core values of dignity, compassion, respect, hospitality, and stewardship. Hospice C.N.A. is part of a collaborative, interdisciplinary team who provides high-quality, person-centered end-of-life care and support including those living at our Kaukauna and Green Bay campuses as well for those living in a 5-county territory. C.N.A. works collaboratively with patients, clinical teams, and family members to ensure physical comfort and personal cares needs are met based on individualized RN assessments and patient-specific needs and goals. C.N.A. provides personal cares to support the physical, psychosocial, and cognitive needs of those we serve.
