Position Summary:

Unison Health is seeking a Substance Abuse Specialist to join our Assertive Community Treatment (ACT) Program, an evidence-based model of delivering comprehensive community-based behavioral health services to adults with serious and persistent mental illnesses who have not benefited from traditional outpatient treatment. The ACT model utilizes a multidisciplinary team of providers who work closely together to deliver services designed to meet clients’ needs, including substance use treatment, vocational support, and coordinated team-based care.

Key Responsibilities & Role Highlights:

  • Provide services to assist in resolving or minimizing the effects of a client's impairment. 
  • Complete assess­ments, monitoring of conditions, and crisis interventions.
  • Provide education, linking and training in community resources to clients.
  • Assistance in daily living skills and help with obtaining necessary services for meeting basic human needs and entitlements. 
  • Participates as a member of the ACT treatment team and serves as a consultant to the team in the area of substance abuse. 

Education & Experience Requirements:

  • QMHS 3+ with high school diploma required. 
  • Bachelor’s degree preferred. 
  • CDCA (can be obtained within 6 months of hire).
  • Experience in integrated dual disorders treatment preferred.
