We are seeking a talented Registered Nurse to join our team. As a Home Health and Hospice RN, you will be responsible for planning, organizing, and directing home care services. You will have the opportunity to work in a community health setting, providing education and support to individuals and families within their homes and communities.

Job Summary:

The Home Health and Hospice RN plans, organizes, and directs home care services and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Duties & Responsibilities:

  • Develop and initiate the plan of care in partnership with the patient, representative, and caregiver(s)
  • Provide preventative and rehabilitative nursing procedures as ordered by the physician
  • Offer patient, caregiver, and family counseling
  • Deliver patient and caregiver education
  • Prepare clinical notes
  • Communicate with the physician and other healthcare practitioners related to the home health plan of care
  • Identify discharge planning needs and implement prior to patient discharge

Communication:

  • Prepare clinical notes and update the primary physician when necessary
  • Communicate with the physician regarding the patient's needs and report any changes in the patient's condition
  • Coordinate care plan with community health-related persons

Additional Duties:

  • Participate in quality assessment and performance improvement programs
  • Participate in on-call duties as defined by the on-call policy
  • Ensure availability of necessary equipment and services
  • Instruct, supervise, and evaluate home health aide care provided every two weeks
  • Supervise Licensed Practical Nurses (LPNs)/Licensed Vocational Nurses (LVNs) and/or Home Health Aids (HHAs)/Certified Nursing Assistants (CNAs), as directed

Job Requirements:

  • Graduated from an approved school of nursing and licensed in the state of practice
  • Minimum of two years of experience, with preference for public health or home care nursing
  • Maintains a current CPR certification
  • Licensed driver with an insured automobile in good working order
  • Self-directed and able to work with minimal supervision
  • Excellent observation, problem-solving, and communication skills
  • Ability to organize and prioritize workload independently
  • No management experience required
  • Able to perform required nursing skills
