Job Title: Services Coordinator

Department:               Day Services
Reports To:                 Area Manager, Adult Day Services
Employment Type:     Hourly, Professional

Pay Grade:                   G-6
Driver Status:             Authorized – DOT Physical Certification Required


About the Role:

Are you passionate about making a difference in the lives of adults with developmental disabilities (DD)? As a Services Coordinator, you’ll help individuals build skills, connect with their communities, and live more independently. You’ll work closely with participants, families, and care teams to create personalized plans that empower each person to reach their goals.


What You’ll Do:

  • Coordinate and manage services for 30+ adults receiving day support.
  • Develop personalized, community-based plans to help participants grow and thrive.
  • Lead team meetings, skill assessments, and regular progress check-ins.
  • Maintain accurate participant records and provide timely follow-up.
  • Be the main contact for families, care teams, and outside agencies.
  • Support participants through group or individual sessions on skills like anger management, communication, and social interaction.
  • Ensure all safety and health guidelines are followed.
  • Provide transportation and assist with personal care when needed.
  • Monitor incidents and ensure accurate reporting and resolution.

Additional Responsibilities:

  • Participate in meetings and training sessions.
  • Help train staff on service plans and behavior supports.
  • Step in to support direct care when needed.
  • Assist with hygiene, transfers, and mobility as necessary.

What We’re Looking For:

  • Preferred: Bachelor’s degree in a DD-related field OR equivalent experience.
  • Valid Ohio driver’s license and personal auto insurance.
  • Ability to pass a DOT physical (or doctor’s clearance) for transportation duties.
  • Certifications required for this position, which must be kept up to date.
  • Passion for supporting people with disabilities and helping them succeed.

Work Environment:

  • You'll work in various community settings, including animal shelters, pantries, and more.
  • Be prepared for driving in traffic or bad weather.
  • Must be comfortable assisting individuals with mobility and behavior needs.
  • Noise levels are moderate, and you’ll spend time both inside and outside.

 


Physical Requirements:

  • Walk, stand, bend, and move throughout the day.
  • Help with mobility and lift (up to 100 lbs. with assistance).
  • Use lifts or mechanical equipment as needed.
  • Frequently use computer and document work.
  • Drive in a variety of traffic/weather conditions
