The Senior Family Support Counselor will provide strength-based supportive services to youth, adults, and families in home and community settings. The Support and Stabilization team provides a range of services funded by the Department of Children and Families (DCF), assisting individuals and families in navigating challenges to promote improved functioning. The Senior Family Support Counselor will hold additional responsibilities in support of colleagues and overall program functioning. This is a hybrid role, with 60-70% of time in person required. Direct services must be delivered in person, while administrative tasks including paperwork may be completed remotely.

- Conduct home-based assessments of strengths and needs, inclusive of the Five Protective Factors Survey.
- Collaborate with families to create a safety plan incorporating any risk issues.
- Develop and coordinate treatment planning.
- Provide individual and family coaching and support.
- Identify community resources and develop natural supports for youth and parent/guardian.
- Provide comprehensive care coordination:
- Attend meetings with schools, hospitals, and other providers
- Maintain regular communication with collateral contacts as indicated
- Provide referrals and linkage to appropriate supports and services
- Coordinate planful discharge from services, ensuring that necessary supports are in place
- Provide coaching in support of decision making in both crisis and non-crisis situations.
- Provide skills training for youth and parent/guardian.
- Provide transportation to youth and parent/guardian as needed in connection with service delivery.
- Conduct member outreach as needed to engage members and families.
- Complete all required documentation in a timely manner.
- Attend and actively participate in supervision and staff meetings.
- Monitor clinical risk; provide appropriate intervention and escalation where needed.
- Develop and maintain working relationships with DCF and other relevant community partners.
- Attend Leadership Meetings as needed to support functions of the team.
- Provide consultation, mentorship, training, and other tasks related to staff and team development.
- Have an active role in onboarding new staff through the orientation process.
- Engage in additional community outreach/events as needed.
- Meet adjusted productivity targets.
- Must complete all trainings as assigned.
- Ensure that all youth, parents, and guardians are treated with dignity and respect in accordance with Advocates’ Human Rights policy.
- Perform all duties in accordance with the agency’s policies and procedures.
- Strictly follow all agency Performance Standards.
- Collaborate with program leadership to develop and implement systems, structures, and resources to support high quality service delivery.
- Bachelor’s degree in social work, psychology or related field, or 5+ years of experience working with youth and/or families.
- Must be proficient in use of standardized documentation procedures and Electronic Health Records.
- Must be able to perform each essential duty satisfactorily.
- Ability to communicate effectively verbally and in writing.
- Strong organizational skills and ability to multi-task.
- Ability to develop and maintain satisfactory working relationships with others.
- High energy level, superior interpersonal skills and ability to function in a team atmosphere.

- Must hold a valid drivers’ license. Must have access to an operational and insured vehicle and be willing to use it to transport youth, parents, and guardians.
