Residential Care Coordinator-Day Shift LPN/RN- Gran Villas-

JOB SUMMARY:

Licensed nurse (RN or LPN) responsible for coordinating patient care specific to meet patient need, and needs for patient outcome, cost, and communication, operating under the role as Assisted Living Operator.

JOB RESPONSIBILITIES:

• Assumes responsibility for coordinating plans for the total care of each resident which comply with physician’s orders, governmental regulations, HCBS billing, and facility resident care policies.
• Confers daily with the facility director regarding current status of nursing services to residents and submits written reports as requested.
• Participates in the recruitment, selection, hiring, orientation, and of nursing personnel
• Assists in controlling nursing service hours and supply costs.
• Maintains schedule for all nursing staff.
• Assures 24 hr. nursing coverage in the facility each day. Works in floor position to fill in for absent nursing staff or to meet resident census staffing needs.
• Assists facility director as requested with measuring facility clinical outcomes.

MINIMUM QUALIFICATIONS AND RELEVANT EXPERIENCE REQUIRED:

• Current registration as an LPN or RN for the state in which the facility is located
• Preferred one year experience in the long term care field
• Preferred supervisory or management role in health care field.
• Possess leadership skills
• Strong written and verbal communication skills
