Deer Meadows Home Health & Support Services is actively hiring a Licensed Physical Therapist Assistant (PTA) with Indirect License in Pennsylvania to support home health patients throughout Philadelphia County.
This is a field-based home health PTA role delivering 1:1 patient care in the home setting. Ideal for PTAs seeking autonomy, flexible weekday scheduling, and consistent caseloads without clinic burnout.

Job Responsibilities
Provide PTA services under Physical Therapist supervision
Implement individualized treatment plans in patient homes
Support patient recovery with mobility, strength, and ADL training
Monitor patient progress and communicate updates to supervising PT
Educate patients and caregivers on safety and home exercise programs
Complete accurate and timely documentation in HCHB EMR
Travel within assigned Philadelphia County territory
Requirements
Active Physical Therapist Assistant (PTA) license in Pennsylvania
Indirect License required
Home health experience preferred (not required)
Strong communication and clinical documentation skills
Ability to work independently in a field-based setting
Reliable transportation within Philadelphia County
