FBS Surrogate Provider

Requirements

  • High School diploma or GED required.
  • Must be at least 18 years old.
  • Must be able to work a flexible, part-time schedule.
  • One (1) year’s volunteer or paid work experience in a human services field (i.e. education, mental health, nursing home or medical environment) .
  • Must have a current, valid driver's license and good driving record.
  • Ability to communicate effectively with families and maintain positive working relationships with individuals receiving services and care team. Must be able to work with diverse groups of people with varying cultural and educational backgrounds.
  • Must be able to complete New Hire Training within 60 days of hire date.

Summary

  1. Essential Job Duties and Responsibilities:
    1. Department Functions: Provides substitute care, respite, personal care, and other approved one-to-one services to adults and children in Family Based Services programs.
    2. Health and Wellness: Implements health and wellness activities for individuals receiving services. Models appropriate nutrition, exercise and healthy lifestyle choices while working with individuals receiving services.
    3. Assignments: Support services are assigned on the basis of need for the individuals served and the availability of the support provider
    4. Supports for Children: Support providers who have successfully completed the required additional CYFD background checks and who have been approved for service support for children by the FBS Division Director and FBS Support Department Manager may be given special assignments for respite and personal care support services with children.
    5. Health and Safety: Ensures and maintains a safe environment for individuals served at all times. Abides by ARCA safety policies and reports any safety concerns to supervisor or family provider immediately.
    6. Financial: May be responsible for individuals' allowance monies. Abides by ARCA policies on financial responsibilities.
    7. Recordkeeping: Completes incident reports, employee timesheets, substitute care / respite logs, personal care logs, medication logs and other record keeping forms as necessary.
    8. Training: Completes all required training within specified time periods and participate in on-going in-service training according to ARCA Policy on Training.
    9. Abuse/Neglect/Emergency Situations: Treats individuals served with dignity and respect at all times. As required by law, reports all suspected instances of abuse, neglect, exploitation, environmental hazard, suspicious injury and/or injury of unknown origin to the appropriate authorities in accordance with ARCA policies and procedures. Responds appropriately to emergency situations.
    10. HIPAA Compliance & Confidentiality: Has full access to Protected Health Information (PHI) to carry out job responsibilities in compliance with HIPAA regulations and ARCA policies and procedures on confidentiality.
  2. Typical Physical, Environmental & Mental Demands:
    1. Routine movements such as walking, sitting, reaching, bending, stooping, squatting, lifting, and carrying. Mobility to observe, supervise, guide individuals physically, help with walking or personal care, or lift/transfer someone in or out of a wheelchair. May need to use physical intervention techniques for challenging behaviors (Mandt System). May need to drive and provide transportation. Support may be provided oneonone, with families, or independently. Expected to assist individuals in home and communitybased activities.
