Discharge Planning Coordinator

Position Overview

The Discharge Planning Coordinator plays a vital role in ensuring safe, timely, and effective transitions of care for patients. This position supports patients and families by coordinating post-hospital services, addressing social and financial needs, and promoting continuity of care.

  • Schedule: Varying shifts (Sunday, Monday, Friday, Saturday)

Key Responsibilities

  • Coordinate discharge planning for patients across all age groups
  • Collaborate with physicians, nurses, and interdisciplinary teams to develop appropriate care plans
  • Educate patients and families on post-discharge care, services, and available resources
  • Connect patients with community agencies, home health services, and support programs
  • Assist with financial, social, and placement needs
  • Conduct follow-up to ensure continuity and effectiveness of care plans
  • Support efforts to reduce length of stay and improve patient outcomes
  • Advocate for patients while supporting their emotional and social well-being

Minimum Qualifications

  • Education: High School Diploma or equivalent – required
  • Preferred Education: Bachelor’s degree in Social Work or related field
  • Experience: Healthcare, case management, or social services experience preferred

Employment Requirements
