Overview
We are seeking a compassionate and skilled Hospice Nurse to join our dedicated team. The ideal candidate will provide high-quality nursing care to patients in hospice settings, ensuring comfort and dignity during end-of-life care. This role requires a deep understanding of hospice and the Hospice philosophy, as well as the ability to work collaboratively with families and interdisciplinary teams.
RESPONSIBILITIES
Perform evaluations for hospice care along with informational visits as needed.
Perform initial assessments and determine hospice eligibility
Establish the initial plan of care in collaboration with physicians and hospice team Care Coordination
Deliver comprehensive nursing care to patients in hospice settings, focusing on pain management and symptom control.
Identify and evaluate the physical, psychological, social, and spiritual needs of patients and their families.
Initiate appropriate interventions and create comprehensive, responsive care plans tailored to each patient's unique needs.
Collaborate with the interdisciplinary team (IDT) to ensure seamless delivery of services to patients and families.
Provide emotional support and education to patients and their families regarding end-of-life issues.
Provide education to patients, families, and primary caregivers on effective care techniques and best practices.
Document patient care accurately in electronic medical records (EMR) systems while adhering to HIPAA regulations.
Participate in case management activities, including discharge planning and coordination of community resources.
Engage in ongoing education regarding hospice practices, infection control, and patient safety.
JOB CONDITIONS
May be stressful working with end of life circumstances.
The ability to access patients’ homes which may not be routinely wheelchair accessible is required. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care.
Physical activities will include, walking, sitting, stooping, and standing and minimal to maximum lifting of patients and the turning of patients.
JOB TYPE: Full-Time
PAY RATE: $50-$52 an hour
HOURS: 10 AM-7 PM
