Job Description

Overview

We are seeking a compassionate and skilled Hospice Nurse to join our dedicated team. The ideal candidate will provide high-quality nursing care to patients in hospice settings, ensuring comfort and dignity during end-of-life care. This role requires a deep understanding of hospice and the Hospice philosophy, as well as the ability to work collaboratively with families and interdisciplinary teams.

 

RESPONSIBILITIES

 

  • Perform evaluations for hospice care along with informational visits as needed.

  • Perform initial assessments and determine hospice eligibility

  • Establish the initial plan of care in collaboration with physicians and hospice team Care Coordination

  • Deliver comprehensive nursing care to patients in hospice settings, focusing on pain management and symptom control.

  • Identify and evaluate the physical, psychological, social, and spiritual needs of patients and their families.

  • Initiate appropriate interventions and create comprehensive, responsive care plans tailored to each patient's unique needs.

  • Collaborate with the interdisciplinary team (IDT) to ensure seamless delivery of services to patients and families.

  • Provide emotional support and education to patients and their families regarding end-of-life issues.

  • Provide education to patients, families, and primary caregivers on effective care techniques and best practices.

  • Document patient care accurately in electronic medical records (EMR) systems while adhering to HIPAA regulations.

  • Participate in case management activities, including discharge planning and coordination of community resources.

  • Engage in ongoing education regarding hospice practices, infection control, and patient safety.

 

JOB CONDITIONS

  • May be stressful working with end of life circumstances.

  • The ability to access patients’ homes which may not be routinely wheelchair accessible is required. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care.

  • Physical activities will include, walking, sitting, stooping, and standing and minimal to maximum lifting of patients and the turning of patients.

 

JOB TYPE: Full-Time 

PAY RATE: $50-$52 an hour

HOURS: 10 AM-7 PM
