Position Title: Chart Reviewer Position Code: ChartRev-6032


Department: Nursing Safety Sensitive: Yes


Reports to: Director/Manager Exempt Status: No


 


Position Purpose:


All KHI employees are expected to perform their respective tasks and duties in such a way that supports KHI’s vision to be among the kindest, highest quality health systems in the country.








 

Key Responsibilities [List of material responsibilities and essentials duties which must be completed in achieving the objectives of the position]









· Performs concurrent chart audits for the purpose of charging, identifying, and correcting charge omissions, documentation deficiencies, and unsupported charges.


· Identifies department charting trends and provides information to leadership team.


· Collects data required according to the regulatory standards.



Qualifications [Statements regarding minimum educational and experience qualifications, required proficiencies with specialized knowledge, computer proficiencies, military service, required certifications, etc.]









Education: High School Diploma or equivalent.


Certifications: Current BLS certification from American Heart Association or American Red Cross.


Skills and Knowledge:


· Ability to utilize resources and work independently.


· Ability to plan and organize work assignments in an efficient manner.


· Effective written and oral communication skills.


· Computer literate, experience in the use of spreadsheets, Internet, and word processing software.


· Ability to develop and maintain effective working relationships with co-workers and all levels of management.



Preferences [Preferred attributes for the position which are not absolutely required in the minimum qualifications (i.e., multi-lingual, master’s degree)]









Experience:


· Previous healthcare experience.


· Previous completion of medical terminology course.
