Job Description

Job Title: Peer Navigator
Schedule: 10 Hours Per Week (Flexible)

Hourly Rate: $18.00 per hour
Reports To: Program Director

Position Overview

The Peer Navigator plays a vital role in supporting program participants by providing guidance, outreach, and care navigation services. Drawing from lived or relevant experience, the Peer Navigator helps individuals overcome barriers to accessing care and social services, particularly those related to HIV and associated risk factors.

Through empathy, shared understanding, and practical support, the Peer Navigator assists participants in connecting to resources, maintaining engagement with services, and building the skills needed to advocate for themselves. This role focuses on empowering individuals to improve their health, stability, and overall well-being.

The schedule for this position requires flexibility to effectively meet the needs of program participants.

Key Responsibilities

  • Conduct needs assessments and provide one-on-one or group support to participants.

  • Conduct outreach and engage individuals living with HIV.

  • Respond to phone inquiries in a professional and compassionate manner.

  • Assist clients with appointment scheduling, transportation planning, financial assistance, and life skills.

  • Provide education on treatment adherence, safer sex practices, harm reduction, and wellness.

  • Serve as a role model for self-care, healthcare engagement, and building support systems.

  • Participate in team meetings and collaborate with Case Managers on participant service plans.

  • Assist during crisis interventions and support case conferences as needed.

  • Maintain accurate records and protect client confidentiality.

Qualifications

Required

  • High School Diploma or equivalent

  • Valid Driver’s License

This position will travel throughout Suffolk County, Long Island.
