Job Description
Care Guide Partners PACE is a part of Hosparus Health. As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people 55 and older with the support they need to stay in the homes they know and love as they age. PACE helps seniors who need nursing home-level care maintain their independence by providing all the medical care and support services they need to continue living comfortably and safely at home.

 

Schedule: Part-Time (2 days per week- 8:30am-5:00pm)

Position Summary:

The Primary Care Physician provides direct primary medical care to participants of the PACE Program in the context of the Interdisciplinary Team (IDT). This position supervises clinical medical care provided by Nurse Practitioners, RNs, and Medical Assistants.

 

Position Qualifications:

  • MD or DO required. Valid MD license in good standing to practice medicine in the State of Kentucky. Fellowship or additional training in Geriatrics preferred.
  • Minimum two years clinical experience required, with experience working with frail or elderly populations strongly preferred.
  • Excellent communication skills, with ability to interact with a wide range of stakeholders, including the IDT Team and external healthcare providers.
  • Flexibility to travel throughout the PACE service area.
