What You Will Do

  • Provide direct care services to residents while following individualized care plans
  • Administer medications in accordance with standards of practice, state regulations, and company policies, including all required documentation, reporting, and follow-up
  • Perform resident assessments as required by state regulations and community policies under the direction of the Director of Assisted Living
  • Observe residents for changes in condition and report any concerns immediately to the supervisor and/or Director of Assisted Living
  • Document all relevant observations, communications with responsible parties, clinical procedures, and daily care by the end of each shift
  • Maintain and update individualized care plans for each resident
  • Perform other duties as assigned

Supervisory Responsibilities

This position reports to the Director of Assisted Living and/or Director of Memory Care and is expected to work independently while exercising sound judgment. This role may assist with the supervision, direction, and evaluation of other associates and may participate in performance reviews and disciplinary actions.

The ideal candidate will have:

  • High school diploma or equivalent (GED) required
  • Current Licensed Practical Nurse (LPN) license in the applicable state required
  • Current CPR certification required
  • At least one year of related experience preferred

Physical Requirements:
Must be able to sit, stand, walk, climb stairs, reach, bend, stoop, crouch, kneel, and perform repetitive movements throughout the day. Must be able to lift up to 25 pounds unassisted and assist with transferring, repositioning, pushing, and pulling residents using proper equipment.
