We are currently seeking a full-time RN Care Coordinator to join our team at our Bigfork Medical clinic. This position is responsible for providing patient-centered, relationship-based care coordination across ambulatory clinics and Health Care Home programs. This position will frequently collaborate with Providers, RNs, Nurse Manager and other nursing personnel. The RN Care Coordinator nurse will operate at the top of their RN license by utilizing advanced clinical nursing skills with discretion and independence.

 

Care Coordination Responsibilities:

  • Serve as the contact point, patient advocate, and informational resource for patient’s care team, family/caregiver(s), payers, and community resources
  • Assess patient needs, plan care, implement interventions, monitor progress, and evaluate outcomes
  • Identify barriers, social needs, and care gaps.
  • Create, update and share individualized care plans to cultivate and support primary care and specialty provider co-management with timely communication, inquiry, follow-up, and integration of information into the care plan
  • Provide patient education, next steps, and safety-net instructions
  • Conduct ER and hospital discharge follow-ups
  • Call patients with abnormal or complex results requiring RN-level education or clarification
  • Ensure closed-loop follow-up for labs, imaging, referrals, and consults.
  • Administer allergy injections, including safety checks, observation, documentation, and allergist communication.
  • Remove sutures during provider visits per policy and clinical criteria
  • Review MyChart messages and triage appropriately
  • Demonstrate strong collaboration with providers, LPNs/MAs, reception, and care teams
  • Use written protocols to triage patients over the phone and in person

Requirements

  • Advanced clinical nursing skills to use discretion and independence while delivering care
  • Maintain a problem-solving mindset and support a culture of safety and accountability
  • Meet defined quality measures and documentation requirements
  • Achieve timely follow-up expectations (ER visits, TOC, results, MyChart).
  • Three years clinical experience working with chronic disease patients or related case management experience, five or more years preferred
  • EMR experience, EPIC preferred
