The Community Health Worker is a frontline liaison between MyMichigan Health and the communities we serve. This position is responsible for serving as a link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. This person will build a trusting relationship with individuals and families in our community and will be a key member of our patient-centered health teams and community collaborations.
Responsibilities:
(50%)*Communicate directly with patients to access social needs. Assist patients and families in removing barriers to meeting social needs by connecting them with appropriate community resources and assist in navigating internal and extern systems.
(25%)*Promote wellness by providing locally and culturally appropriate information to patients.
(25%)*Refer and link to preventive services through health screening and health care information
OTHER DUTIES AND RESPONSIBILITIES:
Support care team needs in provider offices as appropriate to meet payor metrics and billing guidelines.
Assist patients as part of the care team in self-management of chronic illness and medication adherence.
Advocate for patients when needed.
Navigate individuals with the knowledge, skills and expanded resources to obtain healthcare at the right time and right place.
All other duties as assigned.

Required Education:
Education: High School Diploma or GED
Equivalent Experience: Community Health Worker Core Competency certification is required within 6 months of hire or transfer.
Other Information:
EXPERIENCE, TRAINING AND SKILLS:
Community Health Worker certification preferred.
Experience with community health, public health or related field.
Must have strong written and oral communication skills, including preparation and report presentation.
Knowledge of personal computers, PC applications, MS Word, Excel, PowerPoint and Project.
Must be self-motivated, strong problem solver, and detail-oriented.
Effectively communicate with community members, partners and stakeholders.
Communicating health information using culturally appropriate terms and concepts.
PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS:
Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.
Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.
Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.
Overall vision and hearing is necessary with or without assisted device(s).
Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching.
Some exposure to blood borne pathogens and other potentially infectious material. Must follow MyMichigan Health bloodborne pathogen and TB testing as required.
Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.
Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis.
Physical Demand Level: Sedentary. Must be able to occasionally (0-33% of the workday) lift or carry 0-10 lbs.
