Job Description

The Gurwin Family of Healthcare Services is a renowned healthcare provider located on Long Island, NY which offers a full continuum of services. The Nursing & Rehabilitation Center is a 5-star, 460-bed facility providing services for short-term rehabilitation and long-term skilled nursing care, home care programs, a ventilator dependent care, palliative and hospice care, a new Memory Care Unit and on-site dialysis. The Gurwin Family of Healthcare Services also includes the 201-apartment Gurwin Jewish~ Fay J. Lindner Residences assisted living community, and the proposed Fountaingate Gardens independent living community, to be built on the Commack campus.

Our Assisted Living Community is currently seeking a per diem LPN to work every other Friday/Saturday. Our ideal candidate will embody the passion and commitment required to deliver the ultimate care for our residents and continue our reputation of providing the gold standard of excellence in assisted living.

The LPN Supervisor should be caring and compassionate and must possess excellent nursing skills in addition to strong leadership capabilities to continue our reputation of providing the gold standard of excellence in assisted living. NEW GRADS ARE WELCOME!

Responsibilities include but are not limited to:

·         Overall responsibility for the Wellness Office in the absence of the Director of Resident Services.

·         Supervise personal care services rendered by Gurwin Jewish ~ Fay J. Lindner Residences.

·         Implements standards of care and policy and procedures for the Wellness Department.

·         Coordinate and manage all Assisted Living Aides.

·         Apprise family of changes in resident health and behavior.

·         Responsible for control of medication and documentation of medication.

·         Supervise daily shift logs to ensure compliance.

Requirements include:

·         Current valid NYS LPN license with knowledge of geriatric nursing.

·         Must adhere to the Department of Health Regulations as they pertain to Assisted Living Facilities.

·         Previous supervisory experience.

·         Must possess excellent organizational skills and be detail orientated.

·         Ability to work independently in an active Assisted Living Community.

·         Must have exceptional interpersonal skills, coupled with strong relationship building and be an effective leader.

·         Committed to the well-being of our residents and demonstrates compassion and respect for their concerns.
