Job Description

Schedule: Full-Time | Monday – Friday | Day Shift

$80,000-$85,000 Annual Salary

Position Summary

  • Oversees and coordinates clinical nutrition services for residents in a skilled nursing facility.

  • Conducts nutrition assessments and develops individualized care plans.

  • Ensures compliance with federal and state regulations related to food service and clinical nutrition.

  • Promotes optimal resident health, healing, and quality of life through evidence-based nutrition care.

Key Responsibilities

Clinical Nutrition Services

  • Complete comprehensive nutrition assessments for new admissions, quarterly reviews, annual assessments, and significant change evaluations.

  • Develop, implement, and monitor individualized nutrition care plans.

  • Assess and manage residents at nutritional risk, including weight loss, tube feeding, wounds, renal disease, diabetes, and malnutrition.

  • Monitor and document resident weights, labs, intake trends, and nutritional status.

  • Recommend therapeutic diets and nutrition interventions as appropriate.

  • Provide nutrition counseling to residents and families.

Regulatory Compliance & Documentation

  • Ensure compliance with CMS, state regulations, and facility policies.

  • Maintain accurate, timely documentation in the electronic medical record.

  • Participate in state survey preparation and response.

  • Support Quality Assurance and Performance Improvement (QAPI) initiatives.

  • Ensure dietary services meet sanitation and infection control standards.

Interdisciplinary Collaboration

  • Participate in care plan meetings and interdisciplinary team (IDT) conferences.

  • Collaborate with nursing, therapy, social services, and medical providers.

  • Provide education and in-service training to staff on nutrition-related topics.

  • Communicate diet changes and nutrition concerns to appropriate team members.

Food Service Oversight

  • Review and approve facility menus to ensure nutritional adequacy.

  • Evaluate therapeutic and modified texture diets for appropriateness.

  • Monitor meal service for quality, safety, and resident satisfaction.

  • Ensure compliance with food safety regulations and dietary standards.

Qualifications

  • Bachelor’s degree in Dietetics, Nutrition, or related field.

  • Registered Dietitian credential through the Commission on Dietetic Registration.

  • Active state licensure or certification as required.

  • Experience in long-term care or skilled nursing preferred.

  • Knowledge of CMS regulations and MDS process preferred.

Skills & Competencies

  • Strong clinical assessment and critical thinking skills.

  • Knowledge of geriatric nutrition and medical nutrition therapy.

  • Excellent communication and documentation skills.

  • Ability to work independently and manage time effectively.

  • Strong understanding of regulatory compliance in long-term care.
