Description

Lewis & Clark Behavioral Health Services (LCBHS) seeks a highly motivated case manager for our Comprehensive Assistance with Recovery and Empowerment (CARE) Team. This position will carry a shared caseload. It is a temporary summer position* with limited benefits intended to provide support for the CARE team by filling in for staff who are on vacation.

 

The CARE team provides intensive, wrap-around mental health support in the community for individuals with severe mental illnesses, focusing on home visits, medication management, therapy, life skills training (housing, employment, finances), and crisis support, aiming to reduce hospitalization and improve independence through a multidisciplinary, recovery-focused approach.

 

This is a daytime position with some weekend coverage required.

Key Responsibilities

  • Direct Client Care: Conduct home visits and provide community-based support.
  • Therapeutic Support: Provide individual and group skills training, crisis intervention, and substance abuse support.
  • Life Skills Training: Assist with daily living skills, housekeeping, cooking, shopping, and transportation.
  • Vocational support: Assist with vocational training, job placement and support.
  • Case Management: Help with housing, finances (bill paying), employment, and accessing community resources.
  • Team Collaboration: Work closely with psychiatrists, psychologists, nurses, and other professionals.
  • Documentation: Complete assessments, treatment plans, and progress notes.

Requirements

Essential Skills & Qualifications

  • Background in behavioral health (Human Services, Social Work, Nursing, Counseling).
  • Strong communication and interpersonal skills.
  • Ability to build positive relationships with clients.
  • Commitment to recovery-oriented, person-centered care.
  • Flexibility and ability to work in diverse environments.
  • Experience in vocational services or in the substance use treatment a plus.
