Community Based Skill Builder
Position Overview
The UPLIFT Program provides in-home supportive therapy to adults age 55 and older who are experiencing symptoms of depression and anxiety. The Community Based Skill Builder works directly with clients in their homes, delivering services using the evidence-based PEARLS model of care. This role is ideal for someone passionate about supporting older adults, community-based work, and collaborative care.
Qualifications
- Bachelor’s degree in Social Work or a comparable human services field
- Experience working with older adults
- Strong time management, organizational, and follow-up skills
Key Responsibilities
- Provide in-home skill-building services to older adults using the PEARLS model to address depression and anxiety
- Collaborate with clients to complete outcome surveys according to agency timelines and procedures
- Provide community resource referrals and coordinate care with other professionals as appropriate
- Collect data and complete required clinical documentation accurately and within designated timeframes
- Screen and appropriately refer clients who report suicidal or homicidal ideation in accordance with agency risk management policies
- Serve clients with substance use concerns in alignment with agency policies and procedures
- Maintain complete, timely, and accurate client records that meet agency standards
- Meet established productivity expectations
- Coordinate care with therapists on shared cases through consistent communication
- Facilitate well-coordinated referrals to internal programs or external resources when needed
- Engage in team problem-solving to improve service delivery and program efficiency
- Provide assistance to coworkers when appropriate
- Adhere to all agency policies and procedures
- Perform other related duties as assigned
Compensation & Benefits:
- 12-month positions starting at $50,000/year and goes up depending on experience$20/hr

Work Environment:
The Community Based team provides community-based/in-home services that are headquartered in an office environment. Must be able to provide community-based/in-home services a by operating own vehicle. Exposed to a combination of office, school and mental health treatment environments. Exposed to children with behavior problems. May be exposed to shouting and children who are upset.
