Champion Care is seeking an exceptional Marketing Liaison to support our skilled nursing and post-acute care centers across the Greater Milwaukee, WI market. This role will represent multiple Champion Care facilities within local hospitals and healthcare systems, helping patients transition safely and confidently from hospital to post-acute care while building strong referral relationships that drive admissions growth.
This is a high-visibility, relationship-driven role—ideal for someone who thrives in a fast-paced environment, builds trust quickly, and wants to make a meaningful impact on patient outcomes and facility growth.
What You’ll Do (Your Impact)
Build and maintain strong referral relationships with hospital case managers, discharge planners, physicians, and community partners
Conduct clinical and financial evaluations to ensure appropriate patient placement
Coordinate smooth, timely transitions from hospital to Champion Care facilities
Support and guide patients and families throughout the discharge and placement process
Serve as a trusted, knowledgeable resource to hospital partners
Collaborate closely with Admissions, Nursing, Therapy, and Facility Leadership teams
Represent Champion Care with professionalism, urgency, and integrity
Travel locally within the Greater Milwaukee market to support referral development and admissions growth
What We’re Looking For
Experience as a Marketing Liaison, Hospital Liaison, Clinical Liaison, Healthcare Marketer, or SNF Admissions Specialist preferred
Strong knowledge of Medicare, Medicaid, managed care, and hospital discharge planning
Excellent communication, relationship-building, and customer-service skills
Ability to work independently within hospitals and healthcare settings
Highly organized, self-motivated, and results-driven
Passion for connecting patients with the right level of post-acute care
