Summary/

Objective



The Behavioral Health Care Coordinator is a core member of the care team, including the patient’s medical provider and psychiatric consultant, and the larger primary care team. The Behavioral Health Care Coordinator is responsible for supporting and coordinating the mental and physical health of patients, under the guidance of the medical provider and when appropriate, other mental health and substance use providers.   



Essential Functions




  • Scheduling appointments for behavioral health, substance use disorder, and psychiatric providers

  • Provides education and community resources for mental health conditions and addiction management services

  • Coaches and encourages patients in the self-management of their health issues by promoting SMART goals

  • Closing the loop to all behavioral health, substance use, and other psychiatric related referrals to increase access to care

  • Assists with transitions of care from the emergency department or inpatient facilities for patients with behavioral health, substance use disorders, or psychiatric related conditions

  • Provides and explains discharge instructions and patient education. Allowing patients the opportunity to have any questions or concerns answered prior to leaving their appointments

  • Coordinates and provides access to individual and family support services

  • Provides follow up to patients to find out if they are meeting their goals and how to overcome barriers

  • Effectively communicates with the patient’s care team / PCP, to exchange information about the patient, to gain insight into the patient’s clinical challenges and to give updates on progress made with care management and self-management support services

  • Maintains top quality proficiencies across all training areas in daily work.

  • Attends meetings and training sessions as required.

  • Executes other assigned tasks as requested by management.

Work Environment



Work is performed in an ambulatory care environment. Involves frequent personal and telephone contact with patients, physicians, and other healthcare personnel. Work may be stressful at times. Interaction with others is constant and interruptive.



Travel



Travel outside of Northshore locations will be rare for this position.
