We are seeking an experienced Social Work Supervisor to lead our social work and care coordination team with a strong focus on advancing complex, social work–driven cases and optimizing patient progression throughout the continuum of care.
- Supervise, mentor, and support social work and care coordination staff
- Conduct case reviews and provide clinical guidance on complex, high-risk cases
- Lead daily escalation meeting and weekly case progression/long length of stay meetings focused on care coordination and social work-driven barriers
- Monitor productivity, documentation quality, and regulatory compliance
- Participate in hiring, onboarding, and performance evaluations
- Ensure timely psychosocial assessments
- Oversee complex discharge planning including SNF placement, behavioral health transfers, homelessness, guardianship, APS/CPS involvement, and community linkage
- Address barriers to discharge and escalate appropriately to leadership
- Collaborate with utilization management to support appropriate level of care
- Promote patient advocacy and culturally competent practice
- Track and improve metrics related to length of stay, avoidable days, and readmissions
- Identify workflow gaps and implement process improvements
- Ensure compliance with NYS regulations, CMS guidelines, and hospital policy
- Master’s Degree in Social Work (MSW) from an accredited program
- Current New York State LMSW required; LCSW preferred
- Minimum 3–5 years hospital social work experience
- Prior supervisory or leadership experience preferred
- Strong knowledge of discharge planning, community resources, and regulatory requirements
- Demonstrated ability to manage complex, fast-paced clinical environments
USD $43.92 - USD $60.78 /Hr.
