About the Role

We are seeking a compassionate and reliable Home Health Aide to support patients served by Fortis Health–affiliated agencies. In this role, you will provide hands-on assistance in the home, helping patients with daily living activities while supporting their comfort, safety, and independence.

No attendant care is provided (no housekeeping, errands, or companionship).

Responsibilities

• Follow the instructions of the professional nurse/therapist in providing care.
• Provide assistance with personal care (bathing, oral care, and dressing)
• Maintain patient confidentiality and adheres to HIPAA requirements.
• Ambulation, positioning, and turning.
