Job Summary:

We are seeking a compassionate and self-motivated Certified Dementia Practitioner to join our team as a patient advocate for our dementia population. The successful candidate will have reliable transportation and demonstrate initiative in supporting our residents. This role will involve liaising with our internal facilities (Butler, Rosewood, Seasons) to provide education, program development, and support. Key responsibilities include:

- Providing expert guidance and support to residents, families, and staff

- Developing and implementing dementia-specific programs and education

- Collaborating with interdisciplinary teams to enhance resident care

- Advocating for resident needs and preferences

- Building strong relationships with families and healthcare professionals

Requirements:

- Certified Dementia Practitioner (CDP) certification

- Reliable transportation

- Strong communication and interpersonal skills

- Compassionate and patient-centered approach

- Ability to work independently and as part of a team

- referral generation, marketing, conversions

Preferred Qualifications:

- Experience working with dementia populations

- Education or training in dementia care

- Program development and implementation experience
