Description

About the Role
As an Intake Coordinator at CareM Support, you are the front door to care — the first connection in a patient’s journey. In this role, you’ll coordinate referrals and admissions with precision and urgency, helping ensure patients are onboarded smoothly into home-based services.

This is a fast-paced, high-impact position where your ability to communicate clearly, stay organized, and manage competing priorities directly supports patient care. You’ll work alongside a strong, collaborative team that relies on each other to get important work done well — and done right.

At CareM, our mission is simple: to bring out the courage in others. That includes the patients we serve — and the people we work beside every day.

Key Responsibilities

  • Receive and process patient referrals via phone, fax, email, and in-person communication
  • Verify insurance benefits and communicate eligibility clearly to internal teams and referral sources
  • Enter and maintain accurate patient data in EMR systems
  • Coordinate start-of-care details, including patient outreach and physician verification
  • Track documentation and follow up with internal teams to ensure timely completion
  • Collaborate with clinical, billing, and operations teams to support seamless patient onboarding

Requirements

What We’re Looking For

  • 1–2+ years of experience in healthcare intake, admissions, scheduling, or a related role (preferred)
  • Strong communication skills — clear, professional, and responsive
  • High attention to detail, especially with data entry and documentation accuracy
  • Ability to manage multiple priorities in a fast-paced environment without losing focus
  • Basic understanding of insurance verification and healthcare workflows
  • A calm, steady presence — someone who can stay organized and composed under pressure

Why Join Us
CareM Support is the operational backbone for a network of healthcare providers — supporting everything from patient intake to billing and authorizations. You’re not just doing administrative work — you’re helping care happen.

Here, you’ll find:

  • A strong, team-first environment where people support each other and share responsibility
  • A fast-paced setting that keeps you engaged and challenged
  • Growth opportunities within a mission-driven healthcare organization
  • A culture grounded in trust, collaboration, and accountability
  • The chance to be part of something bigger — a system designed to deliver better care, together
