Rehoboth is hiring an experienced Intake Specialist to support our home care operations. The person will be responsible for managing new patient intake and ensuring all information is accurate.
The person will be responsible for communicating with patients and families while supporting coordination, billing, and compliance.
Key Responsibilities
- Answers telephone calls and relays all messages.
- Conduct intake interviews and gather required documentation
- Verify insurance eligibility and coverage
- Actively monitor the incoming intake referrals for potential patients.
- Assists in marketing efforts for potential clients.
- Communicate with the patients, family members, physicians, and discharge planners from other entities such as rehabilitation, hospitals and nursing homes.
- Enter patients’ information into the software system, which includes uploading patient referral documents, clinicals, and all other documents from referrals
- Manage the full intake process for new home care patients; collect and verify patient demographics and insurance information
- Follow up on incomplete referrals and pending documents
- Provide general administrative support as needed
- Run and send monthly reports to the management.
Qualifications
- Required: One (1) to two (2) years of home care intake experience
- Experience with HHA Exchange: preferred
- Familiarity with the ePaces system
- Knowledge of Medicare and Medicaid
- Strong communication and phone skills
- Proficient in Microsoft Office 365
- Bilingual in English and Spanish
