The Opportunity:

  • Position: Physical Therapist (PT)

  • Location: Concord, CA (Contra Costa County)

  • Compensation: Starting at $60.00 per hour (DOE)

  • Availability: Full-Time, Part-Time, and PRN

  • Shifts: Flexible 5-day schedules (Tue–Sat or Sun–Thu) — Skip the Monday rush!

Your Impact & Responsibilities:

  • Clinical Leadership: Perform comprehensive evaluations and develop physician-approved treatment plans tailored to each resident's recovery journey.

  • Team Supervision: Provide direct patient care and expert supervision of Physical Therapy Assistants (PTAs) in accordance with California State Practice Acts.

  • Interdisciplinary Teamwork: Collaborate with the medical and nursing teams to communicate patient progress, goals, and discharge planning.

  • Resident Advocacy: Lead discharge planning and recommend necessary durable medical equipment (DME) to ensure a safe transition home.

  • Education: Provide vital education to nursing staff, patients, and families regarding long-term maintenance and caregiver safety.

  • Documentation Excellence: Maintain compliant, precise, and timely documentation of all clinical services.
