The Full-Time Home Health Admissions Registered Nurse at KeystoneCare plays a crucial role in welcoming patients into the compassionate world of home‑based care. As the first clinical touchpoint, you’ll use your expertise to assess patient conditions, collaborate closely with physicians, and design individualized care plans that support healing, independence, and comfort in the home setting.

In this impactful role, you'll guide patients and families through the start of their home health journey—providing education, reassurance, and clear direction while ensuring every care plan reflects the patient’s unique needs and goals. Your work is essential in promoting patient confidence, empowering self‑care when possible, and creating a strong foundation for exceptional outcomes.

Shift:

Friday, Saturday, and Sunday — 8:00 AM to 8:00 PM

Success in this role requires excellent communication skills and the ability to collaborate seamlessly with our interdisciplinary care team. You will be a key connector, ensuring smooth coordination of services so each patient receives the highest quality of care from admission onward.

Primary Job Responsibilities

  • Evaluates patients for appropriateness for home health services, ensuring regulatory compliance and agency standards are met.
  • Obtains treatment authorization and processes patient admissions to the agency.
  • Conducts comprehensive clinical evaluations to inform care decisions.
  • Collaborates with physicians to communicate patient needs and secure appropriate care orders.
  • Initiates and develops the plan of care based on the evaluated needs of patients.
  • Educates and counsels patients and family members regarding nursing care needs and health management strategies.
  • Performs direct nursing care tasks in accordance with physician orders.
  • Ensures referrals to additional community resources when necessary.
  • Completes Home Health OASIS assessments as appropriate during visit timepoints.
  • Accurately documents all nursing activities within the Electronic Medical Record as per agency policy.
  • Coordinates the dissemination of a written start of care report to all team members involved in patient care.
  • Regularly meets established productivity benchmarks by communicating with supervisors regarding scheduling and assignment adjustments.
