Job Description:We are currently seeking a motivated and experienced Per Diem Occupational Therapist to join our team at a partnered assisted living facility in Westchester, NY. This is a great opportunity to work independently while making a meaningful impact within a supportive, team-oriented memory care environment.
Job Type: Per Diem
Pay: $65–$85 per patient
Location: Briarcliff Manor, NY - Westchester (Assisted Living Facility)
Schedule: Flexible, with required daytime availability
- Patients respond best to daytime treatment sessions
- Must be available to lead a weekly exercise class on Mondays at 10:00 AM

Responsibilities
- Provide 1:1 occupational therapy treatment to residents within the facility
- Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
- Deliver care in accordance with professional standards and the established plan of care
- Lead a weekly group exercise class focused on mobility and functional wellness
- Collaborate with facility staff and caregivers to support patient outcomes
- Plan and implement therapy programs to improve or restore functional abilities, reduce limitations, and enhance quality of life
- Perform and document initial evaluations, progress notes, and discharge summaries in a timely and accurate manner
- Monitor patient progress and modify treatment plans as needed to achieve optimal results
- Provide care with a strong understanding of geriatric and memory care populations
Qualifications:
- Graduate of an accredited Occupational Therapy program
- Licensed (or eligible) Occupational Therapist in New York State
- CPR, AED, and First Aid certifications required
- Prior clinical experience required; candidates must be comfortable working independently
- Experience with geriatric and/or memory care populations strongly preferred
- Strong interpersonal and communication skills
