Shiloh Springs and Carriage Inn, located in Ohio, are seeking a dedicated and flexible Full-Time Physical Therapist Assistant (PTA) to join our rehabilitation team in a split position between both facilities. This is a great opportunity to work in a collaborative, patient-centered skilled nursing environment.

Responsibilities:

• Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist

• Assist in implementing individualized treatment plans to improve mobility, strength, and functional independence

• Accurately document patient care and progress using Net Health

• Communicate effectively with the interdisciplinary team across both locations

• Ensure compliance with all facility policies and regulatory standards

Qualifications:

• Active Physical Therapist Assistant (PTA) license in Ohio

• Experience in a skilled nursing or post-acute care setting preferred

• Experience with Net Health is a plus

• Ability to travel between Shiloh Springs and Carriage Inn

• Strong clinical, communication, and organizational skills

Schedule:

• Full-Time position

• Split coverage between Shiloh Springs and Carriage Inn

• Weekend or holiday rotation may be required
