Job Description

Curry Senior Center was founded in 1972 and is dedicated to helping low income and homeless seniors in San Francisco through a holistic health care approach. Our mission is to promote wellness, dignity and independence for seniors. We accomplish this through collaborative, innovative programs and services.

             

JOB SUMMARY:

This role provides on-going primary care to low income, frail, multiply diagnosed older adults in the Tenderloin/South of Market Neighborhoods. This role is comfortable working in a community-based setting and participates as part of a multidisciplinary team to coordinate the best possible care for patients.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

·        Provide ongoing primary care to low income, frail, multiple diagnosed seniors in the Tenderloin and South of Market neighborhoods. 

·        Identify and manage chronic diseases and acute minor illnesses; provide prevention services; assess for psychosocial needs, communicable diseases, domestic violence and elder abuse, housing problems, and substance use. 

·        Work as a member of a multidisciplinary team. 

·        Participate in meetings and committees. 

·        Keep and prepare documentation as required. 

·        Participate in on-going agency QA/QI projects. 

·        Work in a community-based setting in the Tenderloin serving low income, frail and culturally diverse [including non-English speaking] older adults with mental health issues, substance abuse and chronic health problems. 

·        Support the Recuperative Care Program and provide intermittent coverage for the Recuperative Care Program, including routine patient check-in's and low acuity clinical monitoring to maintain continuity of patient care.

·        Ability to make home visits for homebound patients.

·        Work with support staff in scheduling medical appointments.

·        Other duties as assigned. 

 

JOB REQUIREMENTS:

·        Active RN license in State of California. 

·        Active Nurse Certification RN by the Board of Registered Nurses in State of California. 

·        Minimum of 1-year experience in a primary care setting (i.e., community primary care clinic). 

·        Active BLS certification. 

·        CPR Certification. 

·        Proof of negative or nonactive TB required. Curry Senior Center will provide or reimburse for the cost of a TB test after an offer of employment has been accepted by both parties. Proof of a negative TB test within 90 days will also be accepted.

·        Effective oral and written communications skills with the ability to express ideas in a clear, thorough, concise manner.

·        Strong interpersonal, communication and organizational skills.  Solid aptitude for customer service.

·        High degree of skill in grammar, spelling and punctuation.

·        Ability to maintain the confidentiality of all information obtained.

·        Strong organizational skills and the ability to prioritize workflow.

·        High level of analytical problem-solving skills.

·        Excellent time management skills, including the ability to manage self-development.

·        Must be action-oriented and have the ability to work independently and as a team player.

·        Strong computer skills such as Chrome, Google suite, Apple iOS, Office suite.

 

PHYSICAL REQUIREMENTS/DEMANDS:

The incumbent is in a non-confined office-type setting in which they are free to move about at will. The Environment is mostly clean and comfortable but may include some minor annoyances such as noise, odors, drafts, etc. Work will be performed utilizing a computer, sitting at a desk/chair or standing while speaking and interacting with clients. Position will require average finger dexterity to operate normal office equipment and systems.
