Position Summary:
We are seeking a Full-Time Wound Care focused Nurse Practitioner ready to step into a role that offers clinical ownership, consistent patient relationships, and strong professional growth. You will manage complex wounds, oversee care plans, and serve as a key clinical leader within your communities. This is an ideal opportunity for providers who want to move beyond traditional settings and into a more dynamic, patient-centered model of care.

Responsibilities:
• Provide full-time, on-site rounding including primary care and advanced wound care services.
• Manage complex wounds such as pressure injuries, ulcers, and post-acute conditions.
• Lead chronic disease management and preventative care initiatives.
• Collaborate with physicians, nurses, and interdisciplinary teams.
• Reduce hospitalizations through proactive, hands-on care.
• Communicate with families, caregivers, and facility partners.
• Complete documentation, medication management, and lab coordination.

Qualifications:
• Active Certified Nurse Practitioner (CNP) license in Minnesota (AGNP preferred).
• Experience in wound care, geriatrics, primary care, or post-acute care preferred.
• Valid driver’s license and ability to travel between communities.
• Strong ability to work independently in a mobile environment.
• Passion for delivering high-quality, relationship-based senior care.
