Schedule: Monday–Friday, with evening/weekend on-call rotation as required
This role may begin as a Visiting RN, transitioning into a Case Manager position once a half caseload is assumed, in accordance with the Collective Bargaining Agreement with Montana Nurses Association Local #32. Both roles share the same clinical responsibilities.
Essential Duties and Responsibilities
Provide skilled nursing care and coordinate home-based services based on comprehensive assessments and physician-authorized plans of care
Assess, monitor, and evaluate patients’ physical and psychosocial status and adjust care interventions accordingly
Supervise and evaluate care provided by Home Health Aides and CNAs
Communicate effectively with physicians, interdisciplinary team members, and community resources to ensure continuity of care
Educate and support patients and families through health teaching, counseling, symptom management, and referrals
Document all patient care accurately and in compliance with agency standards and regulatory requirements, including OASIS/HIS/HOPE
Maintain working knowledge of Medicare reimbursement rules and compliance guidelines
Participate in interdisciplinary scheduling and rotate into the on-call schedule to address patient needs via phone triage or in-person visits
Engage in agency quality improvement initiatives and meet established quality metrics
Meet caseload and productivity expectations aligned with agency standards
Attend required in-service training and perform other duties as assigned
Requirements
Qualifications
Current Registered Nurse (RN) license in the State of Montana (BSN preferred)
Valid driver’s license, reliable transportation, and appropriate auto insurance
Two or more years of recent clinical nursing experience preferred
Experience in acute care, home health, or hospice preferred
Ability to perform skilled nursing tasks including venipuncture, catheter care, wound management, and parenteral therapy
