Citrus Heights Post Acute is looking for a dedicated and compassionate Speech-Language Pathologist (SLP) to join our team! You’ll play a vital role in helping patients regain communication, swallowing, and cognitive function in a collaborative, supportive skilled nursing environment.

What You’ll Do
- Evaluate patients within 24 hours (or 1 business day) of physician referral
- Develop and implement individualized treatment plans
- Provide therapy per physician orders
- Monitor, document, and adjust patient progress as needed
- Collaborate with physicians, nursing staff, and interdisciplinary team members
- Participate in care conferences, rehab meetings, and discharge planning
- Educate patients, families, and caregivers for continued care
- Provide in-services and training for facility staff
- Maintain accurate documentation using EMR systems
Qualifications
- Master’s or Doctorate degree in Speech-Language Pathology
- Active California SLP license (required)
- Strong clinical, communication, and documentation skills
- Skilled nursing or post-acute experience preferred, but not required
- Team-oriented with the ability to thrive in a fast-paced environment
Certifications & Requirements
- Current SLP license in good standing with the state
- Ongoing continuing education per state requirements
- Familiarity with EMR systems
